-Franklin Community Health Assessment: 2025: A Community Health Assessment (or CHA) is a health assessment process that identifies important local community health needs through comprehensive data collection and analysis. The Franklin Health Department completes a CHA once every three to five years. Our current CHA begins in 2025 and will end with a final report presented to the community in 2026. -Franklin Health Department 2024 Annual Community Report: An annual overview of the activities, programs, and services offered by the Franklin Health Department.
-Franklin Community Health Improvement Plan - 2023-2025: This is a health plan that is updated every 3-5 years with input from the community, to address health issues and enhance the overall health of the community.
-Franklin Community Health Assessment 2021-2022: A report completed every 3-5 years that reviews comprehensive information about the Franklin community’s current health status, needs, and challenges, while also identifying areas for improvement.
-Franklin Health Department's Internal Three-Year Strategic Plan (2025-2028): An in-depth plan detailing how the Franklin Health Department aims to fulfill its mission and improve public health within the community.
