Camera Registry Program Form

Residents and business owners can be part of the Franklin Camera Registry Program by registering their video surveillance systems with the Franklin Police Department as a valuable tool in an effort to combat and solve crime. It is important to note that the Police Department will NOT have direct access to your surveillance system but will simply request your help with any potential video evidence.

Complaint Form

Do you have a complaint? Residents may notify the City of any type of complaint by filling out this form. (Please Note: In order for this form to be processed, all fields must be filled in. Complaints sent using "Anonymous" as a name and address will not be processed. Please also note, a complainant's name and identification information on a complaint form may be released in response to an open records request for complaint(s).)

Employment Application

The City of Franklin requires anyone who is applying for a position with the City to fill out an Employment Application.

Fire Department Tour/Safety Talk Request Form

Please use this form to request a tour and/or safety talk with the Franklin Fire Department.

Fire Protection Permit

A Minor Modification Fire Protection Permit is required from the Franklin Fire Department for all minor modifications to existing fire alarm and sprinkler systems as defined as an alteration to or addition of 20 or fewer sprinkler heads to an existing automatic fire sprinkler system or 20 or fewer devices to an existing fire alarm system.

Notification of Mailing Address Change

Used to notify the City of a change in mailing address for Tax Bills and Assessment Notices only.

Odor Complaint Form

Quarry Event/Complaint Form

Specific quarry event concerns should be reported to the Quarry Representatives as soon as possible after the event. Use this form to report an event at the quarry or to file a complaint regarding the quarry to the City and/or Quarry Representative. Please Note: A complainant's name and identification information on a complaint form may be released in response to an open records request for complaint(s).

Request for Property Review

Property owners who feel that their assessed value is incorrect can fill out and remit this form to the Assessor's Office to have their property data and assessed value reviewed.

Special Needs Registry Form

The Franklin Police Department recognizes that some members of our community may have special needs, or may react differently when coming into contact with police officers or firefighters during an emergency situation. Information provided will assist emergency responders in providing a more informed response to those with special needs during an emergency.
